What is an NDIS support plan?
A support plan is the operational document that bridges a participant's NDIS funding plan and the day-to-day delivery of their supports. Where the NDIS plan sets out the goals and the total budget, the support plan translates that into specifics: what support will be delivered, when, by whom, and how it connects to the participant's goals.
A good support plan is not a compliance document — it is a working guide that any support worker can pick up and understand. It tells them who this person is, what matters to them, how they like to communicate, what they want to achieve, and what their specific support needs are.
The difference between a support plan that gets used and one that sits in a filing cabinet is specificity. "Participant requires assistance with personal care" tells a worker almost nothing. "Participant showers before breakfast, prefers the handheld showerhead, and likes the bathroom warm before undressing — she will direct the process, worker assists only where asked" tells them everything they need.
Under the NDIS Practice Standards, registered providers are required to develop and maintain support plans in consultation with participants. Plans should be reviewed at least annually, or when a participant's circumstances change significantly.

Part 3 — Support Needs
For each area of support, describe the participant's specific needs, the level of assistance required (full assist / partial assist / prompting only / independent — monitor), and any preferences or approaches support workers must follow.
Personal care (showering, hygiene, grooming, dressing): level of assistance / details and preferences
Meal preparation and eating: level of assistance / dietary requirements and allergies / details
Mobility and transfers: mobility aids used / transfer requirements / manual handling plan attached (yes / no / not required)
Medication (if in scope): whether medication assistance is required / medication management plan attached (yes / no)
Community access and participation: activities supported / transport arrangements
Domestic assistance: tasks in scope / frequency
