Recording a SIRS incident and tracking its notification deadline
Last updated
Choose the SIRS incident type, set Priority 1 or Priority 2, read the Notification Due date, and record when the notification was made.
The short version
Open the incident report, set ACQSC SIRS Priority and Teiro fills in Notification Due: 24 hours for Priority 1, 30 days for Priority 2. After you notify, record Date/Time Notified and the reference number. Teiro does not lodge the notification. You do that in the My Aged Care provider portal.
Choose the incident type
Teiro's Incident Type list includes the eight Serious Incident Response Scheme (SIRS) types:
| SIRS reportable incident | Incident Type in Teiro |
|---|---|
| Unreasonable use of force | Unreasonable use of force |
| Unlawful sexual contact or inappropriate sexual conduct | Unlawful Sexual Contact |
| Psychological or emotional abuse | Psychological or emotional abuse |
| Unexpected death | Unexpected Death |
| Stealing or financial coercion by a staff member | Stealing or financial coercion by a staff member |
| Neglect | Neglect |
| Inappropriate use of restrictive practices | Restrictive practice without consent |
| Unexplained absence during delivery of care | Unexplained absence during care |
Picking a type records what happened. It does not decide whether the incident is reportable. That is a judgement for a person. The SIRS guide and Is this reportable? help you make it.
Set the priority and read the deadline
- 1.Open the incident report. Carers raise reports from the app. See Submitting incident reports.
- 2.Under Funding & Regulatory, turn on Reportable to ACQSC if it is reportable.
- 3.Set ACQSC SIRS Priority to Priority 1 or Priority 2. Carers do not see this field.
- 4.Read Notification Due. It fills in by itself.
| Priority | Notification Due |
|---|---|
| Priority 1 | 24 hours |
| Priority 2 | 30 days |
Two things to know:
- The date comes from the priority. Turning on Reportable to ACQSC without setting a priority does not produce a due date.
- Teiro counts from when the report was created in Teiro. The scheme counts from when your organisation became aware of the incident. If the report was created later than that, the date Teiro shows is later than your real deadline. Create the report as soon as you know.
For the rules behind the two priorities, see Priority 1 vs Priority 2 and When the clock starts.
Record the notification
Teiro records the notification. It does not send it. After you lodge it in the My Aged Care provider portal:
- 1.On the incident report, enter Date/Time Notified. It sits in the NDIS Notification section.
- 2.Under Funding & Regulatory, enter ACQSC Notified At and ACQSC Reference Number.
Once Date/Time Notified is filled in, Notification Due stops changing. A later edit to the report cannot rewrite whether the deadline was met.
NDIS incidents
NDIS incidents work on a different scheme with different timeframes. An incident with no SIRS priority keeps the flat 24 hours from when it was reported. See SIRS vs NDIS reportable incidents.
