Filing a Reportable Incident From a Visit

How a support worker files a reportable incident in Teiro straight from the visit it happened on: the client and shift already linked, a short form to classify and describe what happened, and a save that timestamps and attributes the report instantly.

Report it right from the visit

Sarah is on Harold Nguyen's personal care visit. Before anything else, the shift detail screen makes it unambiguous which client and which service any report belongs to: name, category, and a Report Incident icon sitting in the same record action bar as the rest of the shift's controls, not buried in a separate menu.

The client and shift are already linked

Tapping it opens a dedicated Incident Report form, and two of its fields are already filled in: Participant Involved shows Harold Nguyen, and Related Shift/Booking shows the exact visit Sarah is on. Neither was typed. The record knows which shift the report came from and prefills the participant from it, so the two things a coordinator most needs to identify are never left to a busy worker's memory.

A few taps to classify what happened

Severity and Incident Type are short, single-choice pickers, not free text. Sarah records the incident as Minor and an Injury: two taps, not a paragraph.

Write it up while it is fresh

The Description and Immediate Actions Taken fields are where the specifics go: what happened, and what was done about it, in Sarah's own words, right after the visit rather than reconstructed later from memory.

Filed, timestamped, and attributed

The moment Save is tapped, the incident exists as its own record: Reported By shows Sarah Chen, Date of Incident shows today, both filled automatically. Nobody has to remember to note who filed it or when. Teiro already knows.
