What this recording shows

This is a roughly one minute screen recording of an aged care provider recording a serious incident in Teiro, the care management platform used by Australian NDIS and aged care providers. The Serious Incident Response Scheme, known as SIRS, is the Australian Government's scheme for aged care providers to report serious incidents to the Aged Care Quality and Safety Commission, or ACQSC. Teiro records the incident and works out the deadline. The provider lodges the notification itself in the My Aged Care provider portal. It was recorded on a desktop browser, on the real product, using a demonstration organisation and a made-up client.

Start a new incident report

In Incident Reports, the plus button starts a new report. The first fields are the incident title, the date and time it was reported, and the incident date and time.

Choose a SIRS incident type

The Incident Type list includes the SIRS reportable incident types, such as Unreasonable use of force, Psychological or emotional abuse, Stealing or financial coercion by a staff member, Neglect, Unexplained absence during care and Restrictive practice without consent. Choosing a type records what happened. It does not decide whether the incident is reportable. That is a judgement for a person. In this recording the type is Neglect.

Set the funding context and ACQSC priority

Under Funding & Regulatory, Funding Context is set to Aged Care. The provider then turns on Reportable to ACQSC and sets ACQSC SIRS Priority to Priority 1. Priority 1 incidents must be notified to the ACQSC within 24 hours. Priority 2 incidents have 30 days. Saving the report adds it to the list.

Read the Notification Due deadline

Opening the saved report shows Notification Due, filled in by Teiro. For Priority 1 it is 24 hours after the report was created in Teiro. The scheme itself counts from when the provider became aware of the incident, so the report should be created as soon as the incident is known.

Record the notification

After lodging the notification in the My Aged Care provider portal, the provider opens Edit record and enters Date/Time Notified, then ACQSC Notified At and the ACQSC Reference Number from the portal, and saves. Once Date/Time Notified is filled in, Notification Due stops changing, so a later edit cannot rewrite whether the deadline was met.
